Home Health and Hospice Connection Call – Transitions From Acute to Post-Acute Care

9:00 AM PT / 12:00 PM ET / 5:00 PM London – Transitions from acute to post-acute care are critical moments in the patient and family experience. When communication, coordination, or continuity breaks down, patients and families can be left navigating uncertainty at an already vulnerable time. Join the Home Health and Hospice Workgroup for a connection call focused on strengthening care coordination and improving transitions across the care continuum.

Participants will hear from panelists representing both acute and post-acute settings, as they explore the challenges and opportunities surrounding transitions of care. The conversation will examine communication within care teams and with providers, coordination across care settings, and what an ideal transition looks like from the patient and family perspective.

Panelists will share common pain points, lessons learned from patient feedback, and practical approaches that have helped improve continuity and the overall experience of care. The discussion will also highlight best practices that participants can consider applying within their own organizations.
