A Dialogue on Improving Patient Experience throughout the Continuum of Care
This paper provides us an insight into the active and engaging community dialogue on patient experience across the continuum of care from Patient Experience Conference 2015. Joined by three visionary and thoughtful leaders across healthcare, the dialogue was intended to help us understand and explore the shared ideas, experiences and practices at various points across the continuum. It also reinforced an important point: patient experience is a whole healthcare conversation, issue and opportunity.
