Unfortunately, many patients and family members find themselves in a difficult transitioning position when returning home after a significant health event. They are often left to manage the complexities of care coordination and transition management without support. This webinar will review an approach that hospitals and skilled nursing facilities have used to manage patient transitions and coordinated care. Participants will learn ways to help patients self-advocate by ensuring recognition of early warning signs, as well as a patient’s willingness and ability to seek help when needed. Participants will also grasp an understanding of the common pitfalls in patient transitions and how to avoid them, along with tools and techniques to support the process.
