Committee Charter
- I have received a copy of the Conference Planning Committee Charter and understand the purpose and expectations of this volunteer role.

Meeting Participation
-This committee will meet virtually three times per year and in-person at Patient Experience Conference. I will do my best to attend all meetings. In addition, I agree to provide timely responses to electronic inquiries for feedback between meetings and to participate in additional committee work groups (optional) if available.

Patient Experience Conference
-Attendance at ELEVATE PX is required (In-person or Virtual) for committee members. If I am able to attend the event, I acknowledge that I am responsible for my own travel and lodging expenses. As a committee member, I will receive complimentary conference registration.

Confidentiality
-In accepting this volunteer role with The Beryl Institute, I may be exposed to information which is confidential and/or privileged and proprietary in nature. It is the policy of The Beryl Institute that such information must be kept confidential both during and after term of volunteer service.
